You make a difference in someone's life! Your day starts with a strong espresso in the work café, and you are looking forward to the day ahead. Your colleagues greet you cheerfully. Together, you discuss a number of complex applications for a Wmo facility this morning, and then you get to work on the outstanding caseload. In between, you receive a phone call; a resident thanks you for your support with an application. You accept the compliment with a smile. The afternoon is devoted to kitchen table discussions.
What will you do?
As an experienced Wmo consultant, you are an important source of support for people who want to continue living at home. With your knowledge and commitment, you help them get the right assistance and arrange everything quickly and smoothly. In this way, you help them remain independent and improve their lives. You do all kinds of things: you review applications, give advice, create customized plans, identify problems, maintain contact with clients, informal caregivers, and professionals, and conduct informal kitchen table discussions.
You will become part of our Area Team. The Area Team is an independent team of professionals from various disciplines. We enjoy working in an integrated manner within the broad social domain and are a familiar sight in the villages. We work in an outreach capacity, proactively approaching residents and encouraging them to draw on their own strengths. We work systematically based on the principle of 'one family, one plan, one coordinator.'

- Attractive terms of employment; an attractive salary in accordance with scale 9 of the collective labor agreement for municipalities (max. €4,811 gross per month for a 36-hour working week), an Individual Choice Budget (IKB) of 17.05%, travel allowance for commuting, a pension scheme through ABP, and discounts on various insurance policies.
- A permanent position. Depending on your current situation, you will start with a fixed-term contract.

- You are familiar with the social services in our region and are open to occasionally working outside office hours, such as attending village meetings.
