Request name change
Name use is the right a person has to use the surname (family name) of the (former) spouse or registered partner. You can have your name use recorded by the municipality in the Basic Registration of PersonsBRP). This involves daily use of the partner's surname. On official documents, such as your passport, identity card or driver's license, your own surname remains.

Requirements
In naming, you can choose between the following forms:
- only one's own gender name;
- only the partner's sex name;
- partner's family name, followed by the partner's own family name;
- one's own sex name, followed by one's partner's sex name.
You can change the use of the name at any time after marriage, partnership or termination.

Are you a resident of Opsterland and want to change your name use? Then complete the form "Request change of name use" (at the top of this page under the heading 'Directly arrange'). The municipality records your name use in the Basic Registration of PersonsBRP). The BRP passes this on to other government agencies, such as the Tax Office, pension funds, the water board and the National Road Administration (RDW).
