How the Area Team works
When you register with the Area Team, there will be a period of interviews. During these interviews, we will get a clear picture of your situation. A file will be created and, if help is needed, a plan will be drawn up. On this page, we will tell you exactly what you can expect and how we work.
1. Initial interview
A member of the Area Team will schedule an appointment with you for a meeting. During this meeting, we will discuss the following questions:
- What do you need help with?
- What can you do yourself with the help of others, such as friends, family, neighbors?
- Do you already have help, and if so, what kind of help?
- What are the possible solutions?
We ask that you have a valid ID card or passport with you at the first meeting. We are legally obliged to verify the identity of the people to whom we provide assistance.

2. Follow-up conversation

After the initial interview, we will schedule a follow-up interview. You may find it helpful to bring someone you know well with you. This person (
) can be a family member, friend, or other acquaintance. You can also ask for help from an independent client support worker. They are trained to help you explain your situation clearly. This support is independent of the municipality and is free of charge. For more information, visit

3. File / support plan
After the interviews and any further investigation, we will create a file. If assistance from the municipality is required, the Area Team employee will draw up a report. We call this the support plan. Please read this carefully, as it explains how we will continue to help you and what (specialist) assistance is required.
We only record information about you that is important for assessing what help you/your child needs. This means that sometimes not everything that has been discussed is included in the support plan. You can rest assured that we will treat your data with care. We protect it and only share it with others if you give your written consent.

Good to know
The Area Team works with the Referral Index. This is a national system. The Referral Index shows who is involved with a child, young person, adult, or family. Knowing this about each other enables care providers to work together more effectively. Health support are thus better coordinated. For more information, visit www.verwijsindexfryslan.nl.
