This is how the Area Team works

If you apply to the Area Team, a period of interviews will follow. During these conversations we will get a good picture of your situation. A file will be made and if help is needed, a plan will be made. On this page we tell you exactly what to expect and how we work.

1. First call

An Area Team staff member will make an appointment with you for an interview. In that meeting, we will discuss the following questions:

  • What do you need help with?
  • What manages itself with help from others, such as friends, family, neighbors?
  • Do you already have help and if so what kind of help?
  • What are the possible solutions?

We ask that you have a valid ID or passport on hand at the first interview. We are required by law to establish the identity of the people to whom we provide assistance.

2. Follow-up

After the initial interview, we will schedule a follow-up interview. It may be comfortable for you to bring someone you know well with you at that time. That
can be a family member, friend or other acquaintance. You can also ask for help from an independent client supporter. They are trained to help you explain your situation well. This support is independent of the municipality and is free for you. For more information, visit
https://ocodenktmee.nl.

3. File/support plan.

After the interviews and any further investigation, we create a file. If help is needed from the municipality, the Area Team employee makes a report. We call this the support plan. Read it carefully, because it describes how we will help you and what (specialist) assistance is needed.

We only write down information about you that is important to assess what help you/your child needs. Therefore, sometimes not everything discussed is in the support plan. You can trust us to handle your information with care. We protect them and only share them with others if you give us written permission.

Good to know

The Area Team works with the Referral Index. This is a national system. The Index shows who is involved with a child, youth, adult or family. By knowing this, social workers can work better together. Health and counseling is thus well coordinated. For more information, visit www.verwijsindexfryslan.nl.
